A pain assessment has been added to the Physical Health section of the Assessment document.
With this update:
• The question "Is the client experiencing any pain in the last 30 days?" has been added with Yes/No options.
• When "Yes" is selected, the following fields are displayed:
- Specify where the pain is located and for how long it has been present.
- Obtain client rating from 1 (no pain) to 10 (intense pain).
- Does the client know the cause of the pain? (Yes/No)
- Has the client been treated for pain? (Yes/No)
- List all services client is receiving for pain management, if any.
• When "No" is selected, all pain assessment fields remain hidden.
This improvement enables more comprehensive documentation of the patient's physical condition and pain management.
