The CMS 701 FORM is used to update the progress plan from one period to the next and is completed directly within the appointment in its own tab, just like CMS 700.
Improvements have been added to use existing patient information and simplify form preparation.
- Boxes 1 through 11 are prefilled with the patient’s information, Medicare number, primary diagnosis, and other active diagnoses, so only information that changes for the new period needs to be entered.
- The plan is carried forward from the patient’s most recently completed document within the same discipline, whether it is a CMS 700 or a previous CMS 701, without overwriting information already entered in the new form.
- In Box 8, only one discipline can be selected. Once selected, the start of care date and number of visits are recalculated based on the daily notes for that discipline.
- Boxes in CMS 700 and CMS 701 include guidance explaining what information is expected in each one.
These improvements reduce the need to re-enter existing information and help maintain plan continuity between periods.
